We would be grateful if you would complete this questionnaire about your experience participating in the Green Health Event activity no later than 30th September 2021. Completing the questionnaire should take about 5 minutes maximum.

The information you give us will be treated confidentially and will only be used only for this evaluation. All feedback is warmly welcomed as it is important to us that we get as much insight as possible to allow us to adapt future Green Health activity and provide a project evaluation report.
